This notice explains how the Heart & Health Check handles your information. Version: September 2026.
Taking a test
Your answers are held in the current browser page while you complete the test. They are sent securely to the application to prepare your summary, but are not saved as a patient record unless you submit a booking or callback request. Refreshing or closing the page clears unsent answers.
When you ask us to contact you
We collect your name, mobile number, optional email, answers, test version and consent choices to respond to your enquiry and arrange care. If you arrived through a campaign link, we also retain its campaign label. We do not send your health answers to advertising platforms.
Who can access your information
Authorised reception and care staff can access your details in the private dashboard. Hosting and email providers process information needed to operate this service. Google Calendar and the centre’s existing Outlook calendar connection receive appointment times and a reference, not your questionnaire answers or diagnosis.
Marketing is optional
Booking or requesting a callback does not require marketing consent. If you separately opt in to health news and offers, you may withdraw that permission by contacting reception.
Storage and your choices
Access is restricted, patient records are encrypted in the application database, and access is logged. Enquiry details are retained according to the centre’s approved retention policy, 365 days after collection or the appointment, whichever is later. Short-lived backups may contain deleted details until their retention period expires. Clinical records held separately by the care team may have different legal retention requirements.
You may ask reception to access or correct your information, withdraw optional marketing consent, or request deletion where applicable. Contact reception@heartandhealth.co.za or 031 563 0419. You can also raise a privacy concern with South Africa’s Information Regulator.
Essential cookies
A session cookie keeps your visit secure and supports booking protection. This application does not include advertising pixels or third-party tracking scripts.
A questionnaire, not a diagnosis
The check is for adults aged 18 and older and does not replace a medical consultation. If you feel seriously unwell, call 112 or seek emergency care. Do not wait for a booking or callback.
